This will give you the opportunity to take your time and fill out your forms in the privacy and comfort of your home. You can also complete and safely submit your forms electronically. In order to send your forms to us over the web, you will need to do so using the Google Chrome browser. By completing your forms before your dental appointment in New York, with Dr. Barry Sporer or Dr. Bradley Cutler, our team will have the chance to prepare for your visit and begin creating a custom treatment plan designed just for you.
